The McAuliffe PTSA Youth Entrepreneurship Class is a McAuliffe PTSA run program and is covered by the PTSA’s general liability insurance policy. Participating students must be enrolled at McAuliffe Elementary School as full-time students and McAuliffe Elementary School must be their home school. Parents/guardians will be responsible for getting child to and from all McAuliffe Youth Entrepreneurship Class activities. Parents/guardians may need to attend certain Youth Entrepreneurship Class activities as a chaperone. The McAuliffe PTSA Youth Entrepreneurship Class Chairperson will notify parents/guardians when this is necessary. Neither the school staff nor PTSA chaperones are responsible for the child until the child signs in for the Youth Entrepreneurship Class activity. Youth Entrepreneurship Class activities may be canceled due to weather or unforeseen circumstances. Cancellations will be brought to the attention of all participants as soon as possible and makeup events will be arranged if possible. Youth Entrepreneurship Class will not be held on days off of school. The McAuliffe PTSA strives to have our programs and events accessible to everyone in our community. The McAuliffe Parent Teacher Student Association (PTSA) is a non-profit 501c(3) organization on which the school heavily depends for financial, educational and volunteer support. • I understand that Bizzy Brains is an entrepreneurship enrichment program designed and delivered by Entrepology Studio LLC. The program involves creative, team-based learning, project planning, and basic product or service creation to teach the business cycle - from idea generation to marketing and selling at a simulated marketplace. I agree and give permission for my student to participate.
  
  
• I understand that my child’s participation is voluntary. I hereby release, waive, and hold harmless Entrepology Studio LLC, its instructors, staff, volunteers, partners from any and all liability, claims, or demands arising out of participation in the program, including injury, property damage, or loss.
  
  
• In the event of illness or injury, I authorize Entrepology Studio LLC staff or representatives to obtain emergency medical treatment if needed. I understand that every effort will be made to reach me or my listed emergency contact before additional care is provided. I accept responsibility for all medical costs that may arise from such treatment.
  
  
• I have disclosed all known allergies, medical conditions, and relevant health information to ensure my student’s safety. I understand that failure to provide complete information may limit staff’s ability to respond appropriately in case of emergency.
  
  
• I understand that students are expected to behave respectfully and safely during all sessions. Entrepology Studio LLC reserves the right to remove any student whose behavior is disruptive, unsafe, or repeatedly inappropriate. I acknowledge that no refunds will be issued in the event of dismissal due to behavioral issues.
  
  
• I understand that as part of the learning experience, students may handle small amounts of money or materials during the Business Creation Exercise. Entrepology Studio LLC is not responsible for any loss or damage to personal belongings, project materials, or cash handled by participants.
  
  
• I understand that all program materials, tools, and content are the property of Entrepology Studio LLC and are for my student's personal educational use only unless otherwise stated.
  
  
• I understand that registration is confirmed upon payment. 

• I understand that once the Bizzy Brains program has begun, no refunds will be issued, regardless of attendance, participation level, or personal circumstances. I acknowledge that registration secures a seat in the cohort and supports program planning and materials.
  
  
• I understand that Entrepology Studio is not responsible for delays or cancellations caused by events beyond its reasonable control, including natural disasters, public health emergencies, or government restrictions. In such cases, alternative arrangements or credits may be offered in place of refunds.
  
  
• I understand that this agreement shall be governed by the laws of the State of Washington.
  
  
• I certify that all information provided in this registration form is accurate to the best of my knowledge.
